卒中单元对脑梗死患者早期疗效的评价

目的探讨联合卒中单元对脑梗死患者的早期疗效。方法自2004年12月至2007年2月住院的急性脑梗死患者共238例,随机分为卒中单元组(123例)和普通病房组(115例),卒中单元组按照标准治疗和康复训练,普通病房组按常规治疗,观察比较2组患者1月内神经功能评价(NIHSS),生活能力评价(BI),社会功能评价(OHS),病死率,住院时间,住院花费及患者或家属满意度(VAS)。结果治疗1月后卒中单元组神经功能缺损评分、病死率均低于普通病房组(P<0.05),生活能力评价、社会功能评价、患者及家属满意度等指标卒中单元组均高于普通病房组(P<0.05),但住院时间、住院花费不增加(P>0.05)。结论联合卒中单元能在不增加患者经济负担的基础上,减少患者的神经功能缺损评分,降低病死率,提高患者的早期日常生活能力,减轻病残程度,提高回归社会的能力。

The impact of stroke unit on early outcome of cerebral infarction patients

Objective To investigate the early therapeutic effect of combined stroke unit on cerebral infarction patients.Methods From December 2004 to February 2007,Acute cerebral infarction patients(238 cases) in our department were randomly divided into stroke unit group(123 cascs) and general ward group(115 cases).The stroke unit group received standard therapy and rehabilitation training,and the general ward group received ordinary treatment.The National Institute of Health Stroke Scale(NHISS),Barthel Index(BI),Oxford Handicap Scale(OHS),mortality,hospitalization time and the Visual Analogue Scales(VAS) reflected patients or their family members satisfaction were compared between the two groups after a month.Results NHISS and mortality in the stroke unit group were lower than that in general ward group(P<0.05),but BI,hospitalization time,OHS and VAS were all higher in the stoke unit group(P<0.05).Conclusions Compared with general ward therapy,combined stroke unit can reduce neurological defects,mortality and the ratio of illness handicap,improve the early daily activities and restore social abilities on the basis of no adding expenses of cerebral infarction patients.
