经乳晕途径腔镜甲状腺手术的临床应用

目的探讨经乳晕途径腔镜甲状腺手术临床应用的价值.方法回顾性分析2002年8月～2004年6月15例经乳晕途径腔镜甲状腺手术的临床资料,对腔镜甲状腺手术方法的选择、手术的适应证与禁忌证及手术的安全性等问题进行分析.结果全组14例顺利完成手术,手术时间1～2 h,平均100min.1例因出血中转传统手术.无并发症,住院7 d后痊愈出院.1例诊断为甲状腺癌,后行甲状腺全切术,治愈出院.13例随访3～6个月,平均4.5月.无一例复发. 结论经乳晕途径腔镜甲状腺手术是一种安全可行、具有很好美容效果的手术方法.

Clinical application of endoscopic thyroid surgery through the breast areola approach

Objective To evaluate the clinical applica ti ons of endoscopic thyroid surgery by way of breast areola. Methods We retrospectively analyzed clinical data of 15 cases of endoscopic thyroid surgery through the breast areola approach form August 2002 to June 2004. Proble ms on the selection of surgical methods, indications, contraindications and the safety of the procedures were discussed. Results The operation was completed smoothly in 14 cases, with an operation time of 60~120 min (mean, 100 min), whereas a conversion to open surgery was required in 1 case because o f hemorrhage. No complications were seen. The patients were fully freed of the d isease and discharged from the hospital 7 days after the operation. One patient was diagnosed as having thyroid cancer and was cured with a total thyroidectomy. Follow-up for 3~6 months (mean, 4.5 months) in 13 cases found no recurrence. Conclusions Endoscopic thyroid surgery through breast areola appro ach is safe and applicable, with good cosmetic effects.
