周围神经传导速度对糖尿病神经源性膀胱的临床价值

摘    要:目的探讨周围神经传导速度对糖尿病神经源性膀胱的诊断价值。方法采用丹迪公司Keypoint肌电/诱发电位仪分别测定了14例经B超测定残余尿大于50ml的糖尿病患者(膀胱组)及20例经B超测定残余尿为零的糖尿病患者(正常膀胱组)的双侧上下肢感觉及运动神经传导速度、神经传导潜伏时、病程和糖代谢状况。结果与正常膀胱组比较,神经源性膀胱组的感觉及运动传导速度较慢,相应的神经传导潜伏时较长,平均病程较长,糖化血红蛋白(HbA1c)浓度也较高(t=3.095,P<0.05)。结论糖尿病神经源性膀胱患者常常伴有广泛的周围神经病变,且与长期血糖控制不良有密切关系,临床上测定周围神经传导速度及潜伏时对糖尿病神经源性膀胱有辅助诊断价值。

Abstract:Aim To investigate the diagnostic value of peripheral nerve conduction velocity (NCV) in diabetic neurogenic bladder (NB) so as to find out the relationship between peripheral neuropathy and neurogenic bladder in diabetic patients for the aid diagnosis of diabetic neurogenic bladder. Methods The trial included divided into two groups: 14 diabetic patients with residual urine >50 ml as neurogenic bladder group (group A),6 males,8 females, aged 45-80,course of 4-30 years;20 diabetic patients without residual urine as control group (group B),9 males and 11 females,aged 45-74,with a course of 10 d-16 years.The sensory and muscular conduction velocity(SCV and MCV) in bilateral extremities, latency period of nerve conduction(LP), the course of disease and glycometabolism were measured by Keypoint evoked potential instrument in the 2 groups respectively.Results In comparison with group B,group A had slower SCV and MCV,longer LP and the course of disease,and higher concentration of HbA1c(t=3.095, P< 0.05).Conclusion The diabetic patients with neurogenic bladder have usually extensive peripheral neuropathy,which is closely related with long term insufficient blood sugar control.The determination of NCV and LP in extremities are helpful to clinical of diagnosis diabetic neurogenic bladder.
