2型糖尿病视网膜病变患者血栓前状态标志物检测结果分析

目的 探讨糖尿病视网膜病变(DR)患者血栓前状态标志物的改变及其临床意义.方法 根据眼底病变程度将178例2型糖尿病患者分为无DR组(NDR组)、背景型DR组(BDR组)、增殖型DR组(PDR组).以66例体检健康者作为健康对照组(NC组).检测并比较各组受试对象血浆D-二聚体(D-D)、纤维蛋白原(FIB)、组织型纤溶酶原激活剂(t-PA)、血管性血友病因子(vWF)及血清超敏C反应蛋白(hs-CRP)水平.结果 DR组D-D、FIB、vWF及hs-CRP水平高于NDR组和NC组(P＜0.05或P＜0.001);DR组t-PA水平低于NDR组和NC组(P＜0.001).与BDR组比较,PDR组D-D、FIB、vWF及hs-CRP水平升高,而t-PA水平降低(P＜0.05或P＜0.001).结论 血栓前状态标志物D-D、FIB、t-PA、vWF及hs-CRP检测有助于DR的早期诊断及预后判断.

Variation of prothrombotic state markers in patients with type 2 diabetic retinopathy

Objective To investigate the changes of prothrombotic state makers in patients with diabetic retinopathy(DR).Methods 178 cases of patients with type 2 diabetes mellitus were divided into three groups,including non-DR(NDR) group,background DR(BDR) group and proliferative DR(PDR) group according to the examination of fundus.66 healthy subjects were enrolled as normal control group(NC group).Prothrombotic state markers,including D-D dimers(D-D),fibrinogen(FIB),tissue plasminogen activator(t-PA),von Willebrand factor(vWF) and high sensitive C reactive protein(hs-CRP),were detected in each group.Results D-D,FIB,vWF and hs-CRP levels in DR group were significantly higher than those in NDR group and NC group(P<0.05 or P<0.001),but t-PA level was significantly lower(P<0.001).Compared with BDR group,D-D,FIB,vWF and hs-CRP levels in PDR group were significantly increased and t-PA levels were significantly decreased(P<0.05 or P<0.001).Conclusion Prothrombotic state markers,such as D-D,FIB,t-PA,vWF and hs-CRP,could contribute to early diagnosis and prognostic judgement in patients with DR.
