经纤维支气管镜激光联合注药治疗耐多药支气管结核

目的评价经纤维支气管镜局部激光联合注药治疗耐多药支气管结核的疗效。方法经纤支镜局部激光联合注药治疗18例(治疗组)耐多药支气管结核，并与同期经雾化治疗的21例(对照组)病人作对照进行病情及疗效的观察。结果治疗组全部病人痰茵阴转，病灶明显吸收好转，除1例病人外所有病人的支气管结核病灶吸收好转；而对照组18例病人痰茵阴转，16例病人痰结核茵培养阴转，支气管结核病灶吸收好转15例，两组疗效差别明显。结论经纤维支气管镜局部激光联合注药治疗耐多药支气管结核，疗效可靠，操作相对简单，安全，是一种对耐多药支气管结核治疗的有效方法。

Using the laser and local injection via fibrobronchoscopy to treat multi-drug resistant endotracheobronchial tuberculosis

To explore the efficacy of using the laser and local injection via fibrobronchoscopy to treat multi-drug resistant endotracheobronchial tuberculosis. Eighteen patients with multi-drug resistant endotracheobronchial tuberculosis were treated with the Nd3+:YAG laser and local injection of isoniazid, refampin, streptomycin and levofloxcin with excipient in addition to chemotherapy, while twenty-one patients as the controls were treated with ultrasonic atomizing and chemotherapy. At the end of the average 3.3 months after the treatment, the sputum negative rate was 100%, the radiographic recovery rate was 100% and the focus absorption rate 94.44% in the treatment group, all of that were superior to the controls; the severe cicatrical stenosis rate in treatment group and control group during follow-up were 5.56% and 28.57% respectively; no complication and obvious adverse reaction was observed. Conclusion] Treating multi-drug resistant endotracheobronchial tuberculosis using the laser and local injection via fibrobronchoscopy is safe, comparatively simple and reproducible.
