脑出血患者医院感染的相关因素分析

目的探讨脑出血患者并发医院感染相关因素,以便采取有效控制措施. 方法对1999年6月～2003年6月,住院时间>48 h的584例脑出血患者进行医院感染相关因素分析. 结果脑出血并发医院感染74例,感染发生率12.67%,多发生在住院<2周,下呼吸道感染最多,占36.49%,其次为泌尿道占22.97%,病原学检查主要为条件致病菌;侵袭性操作、抗生素预防性应用、血糖、白细胞计数、意识障碍、肢体运动障碍、年龄、住院天数、营养状态与医院感染的发生相关. 结论脑出血医院感染发生率高,应充分重视这些影响因素,予积极有效的防治措施,减少医院感染的发生率,促进患者康复.

Related Factors of Nosocomial Infection in Cerebral Hemorrhage

OBJECTIVE To explore and control the related factors of nosocomial infection in cerebral hemorrhage. METHODS A total of 584 cases inpatients with cerebral hemorrhage who had hospitalized over 48 h in our hospital from Jun 1999 to Jun 2003 were retrospectively studied. RESULTS It was showed 74 cases with nosocomial infection were found among 584 cases. The infected ratio was 12.67%. Nosocomial infection mostly took place within the first 2 weeks after patients hospitalized. Among inpatients with nosocomial infection,the lower respiratory tract infection was the highest (36.49%),the second one was the urinary tract (22.97%). Pathogenic bacteria were mostly the conditional ones. Nosocomial infection was closely related with such factors as the invasive operation,antibiotic prophylaxis,blood sugar,amount of leucocyte,disorder of consciousness,disturbance of motion,age,hospitalization days,condition of nutrition. CONCLUSIONS Nosocomial infection is still a high frequent complication in cerebral hemorrhage and those related factors should be paid attention. It is the most important step for the rehabilitation of cerebral hemorrhage that to effectively control nosocomial infection.
