联用重组生长激素、膳食纤维促短肠大鼠结肠形态代偿

摘    要:目的:探讨联合应用肠内营养(EN)、膳食纤维(diet fiber)和重组生长激素(recombinant growth hormone,rGH)治疗极短肠大鼠,及对短肠大鼠结肠形态学代偿的促进作用。方法:制成切除80%-85%短肠大鼠动物模型,用百普素行肠内营养,并加用diet fiber和rGH。随机分成三组:肠内营养组(EN),肠内营养+膳食纤维组(EF),肠内营养+膳食纤维 生长激素组(EFG),及正常大鼠对照组(CONT)。观察全身营养状况、结肠形态学、结肠上皮细胞DNA和倍体及结肠组织IGF-1和IGF-1受体mRNA含量。结果:术后第15天EFG组体重减轻明显好于EN组并于第21天体重大于术前。EFG组净氮平衡的改善明显好于EF和EN组,血浆蛋白EFG组明显高于EN组。各组间形态学差异非常显著,各实验组结肠黏膜细胞DNA含量和细胞活动周期明显高于对照组,以EFG组最高。各实验组IGF-1mRNA含量比CONT组明显增高,EFG组最高;各实验组IGF-1受体mRNA含量比CONT组明显减低,以EFG组最低。结论:1.短肠综合征时大鼠结肠发生了明显的形态和功能代偿。2.Diet fiber和rGH对短肠大鼠结肠形态学的代偿有明显促进作用,联用效果最佳。

Abstract:Objective: To study the treatment for the short bowel rats by combination of enteral nutrition,diet fiber and re-combinant growth hormone in the promotion of colonic morphological adaption. Methods: Twenty rats were resected 80~ 85% small intestine and randomly assigned to one of three groups: enteral nutrition group (EN), enteral nutrition,diet fiber group(EF),enteral nutrition, diet fiber and recombinant growth hormone(rGH) group (EFG). Another ten normal rats were controls (CONT). Nutritional state,colonic morphology, IGF - ImRNA and IGF - 1 - recetpor mRNA of the colon were investigated.
