老年肾病综合征并发感染的临床研究

目的　探讨老年肾综患者发生感染的情况及危险因素。方法　记录 98例老年肾病综合征患者所发生感染的情况 ,对患者的年龄、是否使用激素、免疫抑制剂、2 4小时尿蛋白定量、血清白蛋白 (Alb)、血肌酐 (Scr)、血红蛋白 (Hb)、免疫球蛋白(IgG、IgA、IgM)等指标与感染进行相关性分析。结果　感染的发生率为 36 73% ,呼吸道感染占 5 2 78%。感染与患者的年龄、病程、使用激素、免疫抑制剂、血Alb、Scr、Hb、IgG、2 4小时尿蛋白定量有相关性 (P <0 0 5 )。结论　老年肾综患者易发生感染 ,以呼吸道的感染最为常见。高龄、使用激素、免疫抑制剂、低蛋白血症、贫血、大量蛋白尿等为老年肾综并发感染的危险因素。

Clinical study of infection in nephrotic syndrome in the elderly

Objective To investigate the situation and risk factors for infection in nephrotic syndrome in the elderly.Method The study included 98 elder nephrotic syndrome patients.The age,the number of patients using of hormone or immuno-suppressive drug were reported while the amount of 24 hour urine protein,the Alb,Cr,Hb,IgG,IgA,IgM serum levels were measured,and their correlation with infection was evaluated.Result The rate of infection was 36.73%,among them,53.78% was infection of respiratory tract.The significant correlation between age,course,the using of hormone or immuno-suppressive drugs,Alb,Scr,Hb,IgG,the amount of 24 hour urine protein and infection was observed (P<0.01).Conclusion There is a high rate of infection in nephrotic syndrome in the elderly,and the most common is infection of respiratory tract.Great age,the using of hormone or immuno-suppressiv drug,hypoproteinemia,anemia,mass proteinuria are the risk factors of infecton in nephrotic syndrome in the elderly.
