放射性125I粒子植入治疗难治性肝癌的临床应用

目的:探讨植入放射性125I粒子治疗难治性肝癌的疗效及临床应用前景。方法:对明确诊断的11例难治性肝癌患者,在CT或B超引导下将放射性125I粒子永久性植入瘤体内进行内放射治疗。2个月后复查CT或MRI,每个月复查肝功能、甲胎蛋白(α-fetoprotein,AFP)评价疗效。结果:11例中完全缓解2例,部分缓解6例,无变化3例,总有效率(完全缓解+部分缓解)为72.7%。8例AFP阳性患者,治疗后AFP均有明显下降,其中2例下降至正常范围。无治疗相关并发症发生。结论:组织间永久性植入放射性125I粒子治疗肝癌,近期疗效显著、操作简单、安全、损伤小,是原发性肝癌特别是难治性肝癌的又一重要治疗手段。

Clinical application of 125I seeds brachytherapy for refractoriness liver cancer

Objective: To investigate the effect of 125I seeds brachytherapy for refractoriness liver cancer,and its perspective of clinical application.Methods: eleven patients with refractoriness primary liver cancer were treated with permanent implantation of 125I seeds under computed tomography(CT) or B-mode ultrasound tomography guidance for brachytherapy.All of the cases had re-examinations of CT and alpha fetoprotein(AFP) two months after implantation to evaluate the clinical effects.Results: Among 11 patients,2 obtained complete remission,6 obtained partial remission,3 no change.The responsive rate was 72.7%.After implantation,all the eight high-level AFP patients’ level of AFP was lower than before treatment,and two of them had decreased to normal range.The treatment didn’t cause any severe complication.Conclusion: permanent implantation of 125I seeds for liver cancer is safe and minimally invasive,further more it has significant short-term effect.It is another important clinical treatment for liver cancer,particularly for refractoriness liver cancer.
