简易膀胱造瘘在经尿道前列腺电切术中的应用研究

目的]探讨简易膀胱造瘘术在行经尿道前列腺电切术(TURP)中的使用效果及意义。方法]选择行TURP治疗的良性前列腺增生症(BPH)患者l23例,随机分为两组,造瘘组在行TURP的同时行简易膀胱造瘘术,术毕当即拔除造瘘管,非造瘘组常规行TURP术。比较分析两组术前病情资料,及手术中、术后各项指标。结果]两组手术均取得成功,膀胱穿刺造瘘均无穿刺孔出血、冲洗液外渗、拔管后漏尿、切口感染等并发症发生。与非造瘘组相比,造瘘组的手术时间短、出血量少,前列腺电切综合征(TURS)及TURS先兆发生率、手术结束时的血清钠值及血红蛋白值低,差异有统计学意义(P﹤0.05)。两组切除的前列腺碎片组织重量、包膜穿孔情况、膀胱冲洗时间、留置尿管时间、术后住院时间及手术过程费用差异无统计学意义(P﹥0.05)。结论]TURP术中应用简易膀胱穿刺造瘘提高了手术效率,扩大了其手术适应证,可以作为常规使用,值得推广应用。
