经纤支镜局部给药对耐多药支气管内膜结核的治疗研究

目的　评价纤支镜局部给药配合全身化疗对耐多药支气管内膜结核的疗效。方法　将纤支镜导入相应的病变支气管 ,清除病灶分泌物后 ,再注入 Kanamycin.Ofloxacin,配合全身化疗治疗耐多药支气管内膜结核38例 ,并与全身化疗的 32例进行对照研究。结果　经纤支镜给药治疗组 3个月时病灶有效率 81.1% ,肺不张机率2 2 .2 % ,均显著优于单纯化疗组的 6 2 .5 %、4 4 .3% (P<0 .0 5 )。结论　经纤支镜给药配合全身化疗耐多药支气管内膜结核的早期疗效优于单纯全身化疗 ,对改善肺功能有重要意义。

A study on the treatment of multi-drug resistant endotracheobronchial tuberculosis with bronchofiberscope by local injection

Objective To stud the efficacy of brochofiberscope and catheter intervention combined with chemotherapy in the treatment of multi-drug resistant endotrache-obronchial tuberculosis. Methods Thirty-eight cases were treated by the injection of Km and oflx, after cleaning up exudation through brochofiberscope and catheter combined with chemotherapy, whereas 32 controls were treated by chemotherapy only. Results By the end of the 3rd months, the sputum tubercle bacillus conversion rate and atelectasis rate were 81.1% and 22.2% respectively for the treated groups, which were better than the control group: (62.5% and 44.3% respectively) P<0.05.Conclusion Treating multi-drug resistant endotracheobronchial tuberculosis by local injection combined with chemotherapy in the earlier period is effective and better than the chemotherapy only. It is significant in ameliorating spirometry.
