ICU气管插管患者胃管安置方法的探讨

摘    要:目的:探讨静脉应用镇静剂和滴入呋麻滴鼻液在ICU气管插管患者胃管安置中的应用效果。方法选择80例需安置胃管的患者,按安置胃管的先后顺序交替进入对照组和试验组,每组40例。对照组采用传统方法安置胃管,试验组采用静脉镇静和滴入呋麻滴鼻液的方法安置胃管,观察2组患者胃管一次性安置成功率及在胃管安置过程中鼻咽部黏膜损伤情况。结果对照组胃管一次性安置成功率为80%,试验组胃管一次性安置成功率为97.5%;对照组胃管安置过程中发生鼻咽部黏膜损伤率17.5%,试验组胃管安置过程中发生鼻咽部黏膜损伤率为5%,2组比较差异均有统计学意义(P<0.05)。结论采用静脉应用镇静剂和滴入呋麻液的方法安置胃管,可增加胃管一次性安置成功率,降低鼻咽部黏膜损伤,提高患者的舒适性,易于被患者接受,值得临床应用和推广。

Discussion on method of inserting gastric tube in ICU patients with tracheal catheter
Abstract:ObjectiveTo investigate the clinical application about the method of inserting gastric tube in ICU patients with tracheal catheter by using intravenous sedatives and ephedrine and furacilin nasal drops.MethodsEighty patients with tracheal catheter were selected for the study and divided into two groups. Forty patients who were in the control group were inserted gastric tube by routine method,and forty patients who were in the experimental group were inserted gastric tube by the method of using intravenous sedatives and ephedrine and furacilin nasal drops. The effects on changes about the intubation onetime success rate and the nasopharyngeal mucosa injury were observed.ResultsThe onetime success rate in the control group was 80%,and rate in the experimental group was 97.5%.The rate of nasopharyngeal mucosa injury in the control group was 17.5%,and the rate in the experimental group was 5%.The difference was statistically significant(P<0.01).ConclusionInserting gastric tube by the method of using intravenous sedatives and ephedrine and furacilin nasal drops can increase the intubation success rate. It also can decrease the rate of nasopharyngeal mucosa injury. It's suitable and easily acceptable to patients. So the method is worth popularizing in clinical practice.
