血管紧张素系统基因多态性与早发冠心病的相关性研究

摘    要:[目的]研究血管紧张素转换酶(ACE)和血管紧张素原(AGT)基因多态性与早发冠心病的关系。[方法]用PCR法检测46例早发冠心病和65例迟发冠心病患者体细胞ACE基因插入/缺失多态性、用PCR-RFLP法检测AGT基因多态性,并与50例健康者对照。[结果]早发冠心病组ACE的DD基因频率分布与对照组比较有显著差异(P<0.05);迟发CAD组与对照组比较,DD基因频率分布无显著差异(P>0.05);总CAD组(包括早发和迟发CAD)与对照组比较,DD基因频率分布无显著差异(P>0.05)。早发冠心病组和迟发冠心病组AGT多态性中M235T频率与对照组相比有差异(P=0.039和P=0.047),联合基因多态分析,早发冠心病组合并ACE-DD和AGT-TT基因型频率显著高于对照组(P=0.006);迟发冠心病组合并ACE-DD AGT-TT基因型频率亦显著高于对照组(P=0.031)。[结论]早发冠心病组与ACE的DD基因频率和AGT的M235T基因频率分布相关,ACE-DD基因型频率与AGT-TT基因型频率在冠心病发病中具有协同作用。

Abstract:[Objective] To investigate the association of the genetic polymorphisms of rennin-angiotensin system and the risk of premature coronary artery disease(CAD).[Methods] By using PCR and PCR-restriction fragment length polymorphism,insertion/deletion polymorphisms of angiotensin-converting enzyme(ACEI/D) gene and polymorphisms of angiotensinogen(AGT) gene were analyzed in 111 patients,46 patients in premature CAD group,65 patients in mature CAD group and 50 in control subjects group.[Results] ACE-DD genotype was more frequent in premature CAD groups than that control subjects group(P<0.05).ACE-DD genotype was not significantly different in mature CAD groups than that control subjects group(P>0.05).ACE-DD genotype was not significantly different in total CAD groups(including premature and mature CAD groups).than that control subjects group(P>0.05)The analysis revealed that the frequency of AGT-M235T is significantly higher in premature and mature CAD groups than in control subjects(P=0.039 and P=(0.047).)The combining analysis improved that AGT-TT and ACE-DD genotypes were significantly higher frequency in premature CAD group and mature CAD group compared with control group(P=0.006 and P=(0.031).) [Conclusion] The distribution of ACE-DD genotype and AGT-M235T genotype associates with premature CAD,and the combined analysis of AGT-TT with ACE-DD allele has a synergistic effect on risk of CAD.
