全脑室出血铸型患者的外科治疗

目的:评价开颅脑室内血肿清除术联合脑室外引流治疗全脑室铸型出血的效果。方法:回顾对比分析行开颅脑室内血肿清除术联合脑室外引流治疗全脑室铸型出血患者62例和侧脑室穿刺外引流治疗全脑室铸型出血患者58例的临床资料。结果:死亡率:脑室外引流组为34.9%,开颅手术组为14.8%,两组差异有统计学意义(P<0.05);GCS计分:5分以下者两组间死亡率差异无统计学意义,5～8分者两组间死亡率差异有统计学意义(P<0.05);Graeb评分:11～12分者死亡率两组差异有统计学意义(P<0.05),8～10分者两组死亡率差异无统计学意义;随访结果分析,生存患者的预后情况两组差异无统计学意义。结论:开颅脑室内血肿清除联合脑室外引流术治疗全脑室铸型出血效果好,可降低死亡率和并发症。

The craniectomy operation of ventricular hemorrhage cast

Objevtive: To evaluate the efficacy of the craniectomy with ventriculopuncture in treatment of massive ventricular hemorrhage casting.Methods: Sixty-two cases of massive ventricular hemorrhage treated by the craniectomy with ventriculopuncture and 58 cases treated by the simple ventriculopuncture were analyzed retrospectively.Results: The mortality of the group treated by simple ventriculopuncture was 34.9%.The mortality of the group treated by the craniectomy with ventriculopunct was 14.8%.The difference had statistical meaning(P<0.05).The mortalities of the two groups had statistical difference when GCS was 5~8(P<0.05) and Graeb was 11~12(P<0.05),but no statistical difference when GCS was less than 5 and Graeb was 8~10.6 months follow-up showed the prongnosis of the two groups had not statistical difference.Conclusion: The craniectomy with ventriculopuncture is effective in treating ventricular hemorrhage cast.It can reduce the mortality and the incidence of complications.
