摘    要:目的探讨分析不同时期的护理干预对妊娠期糖尿病孕妇分娩结局的影响。方法2019年1月—2020年4月,将该院收治的120例妊娠期糖尿病作为该院研究对象,按照干预时间将孕妇分组,妊娠时间不超过30周的孕妇为早期组,妊娠期间超过30周的孕妇为晚期组,两组均实施相应的护理干预,获取两组干预期间的各项数据,采用统计学方法比较分析。结果两组干预前的空腹血糖、餐后血糖以及糖化血红蛋白水平对比,差异无统计学意义(P>0.05);两组干预后的空腹血糖、餐后血糖以及糖化血红蛋白水平均低于干预前,早期组干预后的空腹血糖、餐后血糖水平低于晚期组,差异有统计学意义(P<0.05),糖化血红蛋白与晚期组比较,差异无统计学意义(P>0.05)。早期组的孕妇并发症(产后出血、羊水过多、胎膜早破、产褥感染)发生率小于晚期组,差异有统计学意义(P<0.05)。早期组的新生儿并发症(早产儿、巨大儿、新生儿低血糖、新生儿窒息)发生率小于晚期组,差异有统计学意义(P<0.05)。结论妊娠期糖尿病需早期确诊、早期治疗、早期实施有效护理干预,积极控制患者的血糖水平,改善患者的病情状态,促使孕妇顺利分娩,降低孕妇及新生儿的并发症发生率,优化母婴结局。

Abstract:Objective To explore and analyze the influence of nursing intervention in different periods on the delivery outcome of pregnant women with gestational diabetes.Methods From January 2019 to April 2020,120 cases of gestational diabetes mellitus admitted to this hospital were taken as the research objects of the hospital.The pregnant women were divided into groups according to the intervention time.The pregnant women who were not more than 30 weeks pregnant were the early group.The 30-week pregnant women were in the late-stage group.Both groups implemented corresponding nursing interventions,obtained various data during the intervention period of the two groups,and compared and analyzed statistical methods.Results There was not statistically significant difference in fasting blood glucose,postprandial blood glucose and glycosylated hemoglobin levels between the two groups before intervention(P>0.05);the fasting blood glucose,postprandial blood glucose and glycosylated hemoglobin levels of the two groups after the intervention were lower than those before the intervention.The fasting blood glucose and postprandial blood glucose levels of the intervention group were lower than those of the lace-stage group,the difference was statistically significant(P<0.05),and there was was not statistically significant between glycosylated hemoglobin and the control group(P>0.05).The incidence of complications of pregnant women(postpartum hemorrhage,polyhydramnios,premature lace-stage of membranes,puerperal infection)in the early group was lower than that in the lace-stage group,and the difference was statistically significant(P<0.05).The incidence of neonatal complications(premature infants,macrosomia,neonatal hypoglycemia,neonatal asphyxia)in the early group was lower than that in the control group,andthe difference was statistically significant(P<0.05).Conclusion Gestational diabetes requires early diagnosis,early treatment,and early implementation of effective nursing interventions to actively control the patient's blood glucose level,improve the patient's condition,promote smooth delivery of pregnant women,reduce the incidence of complications of pregnant women and newborns,and optimize mother and infant ending.
