绒毛膜促性腺激素治疗隐睾可能损伤生殖细胞的研究

摘    要:目的:隐睾的治疗最初常采用人绒毛膜促性腺激素(HCG)或者促性腺激素释放激素(GnRH)的治疗方法。临床上多使用人绒毛膜促性腺激素治疗。我们就这些隐睾患儿采用人绒毛膜促性腺激素疗法是否会损伤生殖细胞进行了研究评价。方法:87名隐睾患儿随机分为单纯手术组(45例)和HCG+手术组(42例),HCG+手术组患儿在征得其父母同意前提下于手术前接受HCG治疗,单纯手术组患儿只接受手术治疗。所有患儿均行睾丸下降固定术,同时测定睾丸活检标本的曲细精管横断面精原细胞数目。两组在其他方面的情况相似。结果:单纯手术组的患儿的曲精细管断面精原细胞数目高于HCG+手术组的患儿,两组比较差异有统计学意义(P〈0.01);12例正常的曲细精管横断面精原细胞数目仅存在于单纯手术患儿中。结论:隐睾患儿接受人绒毛膜促性腺激素疗法可能降低生殖细胞数目。

Abstract:objective: Human chorionic gonadotropin(HCG) or gonadotropin-releasing hormone(GnRH) is the first hormone treatment on cryptorchidism,HCG is more often clinically used.To evaluate human chorionic gonadotropin on germ cell in the patients with cryptorchidism.Method:87 boys with cryptorchidism were divided into surgery group(45 cases)and HCG+surgery group(42 cases) in random.Two groups were similar in other ways.All were treated with orchiopexy and testis biopsy.The number of spermatagonia cells in cross section of seminiferous tubule in biopsy specimens were counted.Result: The number of spermatogonia in boys who were surgery group were more than those who were HCG+surgery group(P〈0.01).12 cases with normal quantity of spermatagonia cells were only seen in surgery group.Conclusion: HCG probably decrease the number of germ cell in the patients with cryptorchidism.
