壳聚糖用于乳牙活髓切断的临床研究

目的：研究壳聚糖作为乳牙活髓切断盖髓剂的临床疗效。方法：选取我院儿科合作患儿临床明确诊断为深龋，牙根处于稳定期的乳磨牙75颗，随机分为试验组(壳聚糖组)45颗及对照组(氢氧化钙组)35颗，行活髓切断术。于术后2周、1、3、6、12个月复查，内容包括临床检查、电活力测试及x线检查。结果：临床疗效：壳聚糖组优72．22％；良22．22％；失败5．56％，综合判效成功率为94．44％。氢氧化钙组优50％；良20％；失败30％，综合判效成功率为70%。二者间有显著性差异。异常根吸收，壳聚糖组(13．89％)少于氢氧化钙组(40％)，两组间有显著性差异。钙化屏障壳聚糖组(44．44％)与氢氧化钙组(60％)差别无统计学意义。结论：用壳聚糖做乳牙活髓切断盖髓剂，可保持根髓活力，很少引起牙根吸收，可认为是一种具有较高临床疗效的乳牙活髓切断盖髓剂。

CLINICAL STUDY OF CHITOSAN ON VITAL PULPOTOMY TO DECIDUOUS TEETH

Objective:To study clinical effect of chitosan as pulp capping agent on vital pulpotomy to deciduous teeth.Methods:Seventy five deciduous molars,which were clearly diagnosed as deep caries,whose roots were at stable periods,were selected from the cooperated children patients.Forty five teeth and thirty five teeth were randomly selected as test group (chitosan group) and control group (calcium hydroxide group).Vital pulpotomy was performed in both groups.The contents of reexamination included clinical examination,electric vital test and x ray at 2,4,12,24,48 week intervals after operation.Results:The clinical effect was determined as excellent,good and failure,at 72 22%,22 22% and 5 56% respectively in test group,while there were 50%,20% and 30% respectively in control group.The general success rate was 94 44% and 70% in both groups.There was statistically significant difference between two groups.The root absorption in test group (13 89%) was less than that in control group (40%) and there was statistically significant difference between two groups.There was no statistically significant difference at calcified barrier formation in test group (44 44%) and control group (60%).Conclusion:The chitosan can be used as pulp capping agent on vital pulpotomy to deciduous teeth.It keeps vital at root pulp and causes less root absorption.It is suggested that chitosan can be used as a kind of pulp capping agent on vital pulpotomy to deciduous teeth.
