白介素-8基因多态性与肺结核易感性的配对病例对照研究

目的研究中国汉族人群白介素-8（interleukin-8,IL-8）基因-251位点多态性与肺结核发病的关系,并初步探讨其在肺结核发病中与环境因素的交互作用。方法采用1∶1配对病例对照研究设计,用聚合酶链反应-限制性片段长度多态性（PCR-RFLP）分析检测167对病例和对照的IL-8基因-251位点的基因型分布,对肺结核相关环境因素进行问卷调查,并进行单因素和多因素条件Logistic回归分析。结果 TT、TA和AA基因型在病例组和对照组的分布频率分别为19.8%、51.5%、28.7%和40.7%、47.3%、12.0%,单因素条件Logistic回归分析显示AA（OR=2.171,95%CI：1.394-3.382,P=0.001）和TA（OR=1.599,95%CI：1.071-2.389,P=0.022）基因型在病例组中的频率显著高于对照组;调整环境因素后,多因素条件Logistic回归分析显示AA（OR=1.915,95%CI：1.207-3.040,P=0.006）及TA（OR=1.540,95%CI：1.011-2.346,P=0.044）基因型与肺结核发病仍有显著性关联;AA和TA基因型与卡介苗接种史（P=0.032）以及与吸烟史（P〈0.001）对肺结核发病均有显著的相乘模型交互作用。结论 IL-8基因-251位点AA及TA基因型可能是中国汉族人群的肺结核易感基因型,AA和TA基因型可能与环境因素存在交互作用。

A paired case-control study on interleukin-8 gene polymorphisms and susceptibility to pulmonary tuberculosis

Objective To explore the possible relationship between the-251T/A single nucleotide polymorphisms in the interleukin-8（IL-8） gene of Chinese Han population and pulmonary tuberculosis,and the interaction with environmental factors to pulmonary tuberculosis.Methods A 1∶1 matched case-control study was conducted.The-251 IL-8 T/A polymorphisms were detected in 167 pairs of subjects by polymerase chain reaction-restriction fragment length polymorphism（PCR-RFLP） technique.Information on related environmental factors of tuberculosis were collected using a questionnaire designed by ourselves.Univariate and multivariate conditional Logistic analysis were conducted in the end.Results The genotype frequencies of TT,TA and AA were 19.8%,51.5%,28.7% and 40.7%,47.3%,12.0% in case group and control group respectively.The genotype frequencies of AA（OR=2.171,95% CI：1.394～3.382,P=0.001） and TA（OR=1.599,95% CI：1.071～2.389,P=0.022） were significantly overrepresented in case group respectively in univariate conditional Logistic analysis.The significant association remained after adjusting environmental factors between genotypes AA（OR=1.915,95% CI：1.207～3.040,P=0.006） and TA（OR=1.540,95% CI：1.011～2.346,P=0.044） and the susceptibility to pulmonary tuberculosis.There were interactions of multiplicative model between AA and TA and Bacille-Calmette Guerin（BCG）（P=0.032） or smoking（P〈0.001）.Conclusions The-251 IL-8 AA and TA genotypes might be associated with the susceptibility to pulmonary tuberculosis in Chinese Han population,and there might be interactions between environmental factors and them.
