阵发性房性心动过速的动态心电图分析

摘    要:目的探讨动态心电图对阵发性房性心动过速的诊断意义。方法分析32例伴有心脏疾患发生房性心动过速者,与12例无心脏器质性疾患发生的房性心动过速者作对照,观察房性心动过速发生的昼夜节律并统计短阵性房性心动过速的频率和持续时间、与发作相关的房性期前收缩的偶联间期。结果检出阵发性房性心动过速者44例(11.3%),心脏病组32例检出率13%,对照组12例检出率8.4%(χ2=42.8,p<0.001),两组差异有显著意义。患者房性心动过速以右房房速发生率为高。心脏病组房性心动过速时发作的阵数、房速率及持续时间与对照组比较差异均有统计学意义,而发作时联律差异无统计学意义;昼夜节律变化在心脏病组不明显而在对照组明显。结论动态心电图检测对阵发性房性心动过速的临床诊断,指导治疗和评价疗效有重要意义。

Abstract:Objective To evaluate the diagnostic significance of dynamic ECG in patients with paroxysmal atrial tachycardia. Methods We enrolled 32 cases with heart disease and atrial tachycardia, and 12 cases without organic heart disease but atrial tachycardia, atrial tachycardia had circadian rhythms and statistical short ZhenXing atrial tachycardia frequency and duration, and attack the related atrial period before contraction of the coupling between period. Results We detected paroxysmal atrial tachycardia in 44 patients(11.3%), 32 cases had heart disease(13%), and there are 12 cases(8.4% chi-square=42.8, p<0.001). Patients with right atrial tachycardia are much more than those with left atrial tachycardia. Atrial tachycardia group with heart disease in the attack, the number, array and duration of atrial tachycardia were statistically significant with the control group, circadian of the rhythms patients with heart disease is not obvious in the control group. Conclusion The dynamic ECG is valuable for the clinical diagnosis, treatment and guidance therapeutic to atrial tachycardia.
