糖尿病社区防治及管理模式的设想与实施

目的 探讨适合上海市城市和农村的糖尿病社区综合防治模式和干预策略。方法 在2002年上海市卫生局重点课题《上海市糖尿病社区防治适宜方案的研究》基础上,选择7个城市试点社区和2个农村试点社区作为糖尿病防治试点,开展糖尿病患者和糖尿病高危人群的管理以及侧重点不同的综合干预研究。结果 建立了上海市社区糖尿病患者分级管理模式和上海市居民高危人群筛查模型以及相配套的信息管理系统。结论 已经建立的糖尿病管理系统具有一定的实用性和可操作性,并将进一步在19个区、县推广实施,以验证其可行性和有效性。

The Primary Assumement and Implement of Prevention and Cure and Managing Model in Diabetes Community

Objective To study the integrated mode of prevention and cure, the interference policy which fits the community prevention and cure of diabetes mellitus in the urban areas and suburb of Shanghai.Methods On the basis of the stress-subject of board of health, Shanghai, 2002 "The Study of Feasible Scheme of the Community Prevention and Cure of Diabetes Mellitus in Shanghai", we selected seven pilot communities in urban areas and two in country to make experiments about the prevention and cure of diabetes mellitus, to manage the patients with diabetes and the public of high risk, and to study the integrated interference on different emphasis.Results We've set up the mode of graded management to manage the patients with diabetes in the communities of Shanghai, the filter-model of the high risk population in Shanghai, and we also had the assorted information managing system.Conclusion There is something applied and feasible in the managing system of diabetes we had set up, and it will be extended to the 19 boroughs and counties to validate the feasibility and validity.
