老年动脉弹性与颈动脉粥样硬化相关性研究

摘    要:目的:探讨老年人动脉弹性与动脉粥样硬化之间的关系。方法:本研究入选314例老年人,采用日本产全自动动脉硬化测定仪同步记录两侧肱动脉-踝动脉脉搏波传导速度(baPWV)作为反映大动脉弹性的指标,应用颈动脉内膜中层厚度(IMT)、颈动脉斑块的有无估计动脉粥样硬化的严重程度。结果:合并高血压患者颈动脉斑块的发生率显著高于未合并高血压组(P<0.05);baPWV在颈动脉斑块组与颈动脉无斑块组存在显著差异(P<0.001); baPWV与颈动脉IMT呈正相关(r=0.42,P<0.001);Logistic回归分析表明年龄、PWV与颈动脉斑块密切相关 (P<0.05-<0.01)。结论:动脉弹性是反应血管亚临床病变的指标,与颈动脉粥样硬化的严重程度密切相关,可作为预测颈动脉粥样硬化程度的指标之一。

Abstract:Objective: To investigate the relationship between aortic flexibility and carotid atherosclerosis in the aged. Methods: A total of 314 aged men were selected. Aortic flexibility was assessed by brachial-ankle pulse wave velocity (baPWV) which was measured using an automatic device (Japan). Carotid atherosclerosis was assessed by common carotid intima-media thickness (IMT) and atherosclerotic plaques in the carotid artery. Results: Patients with essential hypertension (EH) had higher incidence of atherosclerotic plaques in the carotid artery than those without EH (P< 0. 05). BaPWV significantly increased in plaque group compared with subjects without plaques in the carotid artery (P<0. 001) . BaPWV were found to have a strong positive association with common carotid intima-media thickness (r = 0. 42,P<0. 001). It was confirmed that age.baPWV were independent determinants of severity of coronary heart disease by Logistic regression analysis (P<0. 05-<0.01). Conclusion: Arterial flexibility is the parameter reflecting subclinical diseases and closely related to severity of atherosclerosis;it ban be a marker of cardiovascular risk.
