ADC值和rADC值在脑星形细胞瘤分级诊断中的应用价值

摘    要:目的探讨ADC值和rADC值在脑星形细胞瘤分级诊断中的应用价值。方法经手术病理证实的36例脑星形细胞瘤患者术前除常规MR I检查外,还做了DW I扫描,着重对肿瘤瘤体及瘤周水肿带的ADC值和rADC值进行了测算和分析。结果低级星形细胞瘤瘤体的ADC值和rADC值明显高于高级星形细胞瘤(P<0.01),且前者瘤周水肿带的ADC值和rADC值也高于后者(P<0.05);星形细胞瘤瘤体的ADC值和rADC值与肿瘤的恶性度存在显著负相关(r值分别为-0.845和-0.648,P值均<0.01);瘤周水肿带的ADC值和rADC值也与肿瘤的恶性度呈负相关(r值分别为-0.607和-0.477,P值均<0.05)。结论ADC值和rADC值在脑星形细胞瘤术前分级诊断中具有重要价值。

Abstract:Objective To explore the value of apparent diffusion coefficient(ADC) and relative apparent diffusion coefficient(rADC) values in the grading of astrocytomas. Methods 36 patients with astrocytomas histologically proved underwent routine and diffusion-weighted imaging before operation.The ADC values and rADC values of the tumoral parenchymas and peripheral edema regions were calculated and analyzed. Results The ADC values and rADC values of the low-grade astrocytomas were higher than those of the high-grade ones in the tumoral parenchymas(P<0.01) and in the peripheral edema regions(P<0.05).The ADC values and rADC values of the tumoral parenchymas were inversely associated with the degree of malignancy of the astrocytomas(the r values were-0.845 and-0.648 respectively,and P<0.01).The ADC values and rADC values of the peripheral edema regions were also inversely associated with the degree of malignancy of the astrocytomas(the r values were-0.607 and-0.477 respectively,and P<0.05). Conclusion ADC values and rADC values play an important role in the preoperative grading of astrocytomas.
