磁共振FSPGR技术在血管性三叉神经痛诊断中的应用研究

摘    要:目的利用磁共振FSPGR血管成像技术研究三叉神经痛与责任血管之间的关系。方法选取80例原发性三叉神经痛患者,其中2009年5月前收治的患者40例(A组),采用三维时间飞跃法(3D-TOF)磁共振血管成像;2009年5月以后收治的患者40例(B组),采用3D-TOF及FSPGR序列磁共振血管成像。比较两组患者影像学检查诊断符合率,分析三叉神经痛与责任血管之间的关系。结果 A组患者3D-TOF检查诊断符合率为82.5%(33/40),B组患者采用3D-TOF及FSPGR序列检查诊断符合率为97.5%(39/40),两组诊断符合率间差异有统计学意义(P<0.05)。B组FSPGR序列磁共振血管成像明确责任血管依次为小脑上动脉、基底动脉、椎动脉、小脑前下动脉等。结论磁共振FSPGR序列血管成像是显示三叉神经与血管关系最有效的无创检查方法。

Abstract:Objective To evaluate the relationship between trigeminal neuralgia and offending vessels by the three-dimensional fast spin echo(3D-FSPGR)magnetic resonance tomographic angiography.Methods 80 patients with trigeminal neuralgia were divided into group A(40 patients admitted before May 2009) and group B(40 patients admitted after May 2009).3D-TOF was used in group A and 3D-TOF and FSPGR were used in group B.The angiography results were compared between the two groups and the relationship between trigeminal neuralgia and offending vessels was analyzed.Results Coincidence rate of 3D-TOF in group A was 82.5%(33/40) and the coincidence rate of 3D-TOF and FSPGR in group B was 97.5%(39/40),the difference between the two groups was statistically significant(P<0.05).FSPGR confirmed the offending vessels in group B,which were superior cerebellar artery,basilar artery,vertebral artery and anterior inferior cerebellar artery.Conclusion FSPGR magnetic resonance tomographic angiography is the most effective noninvasive method to show the relationship between trigeminal neuralgia and offending vessels.
