摘    要:目的研究神经生长因子(NGF)对脑出血(ICH)患者血清中神经元特异性烯醇化酶(NSE)水平的影响及临床意义。方法选择出血量10~30mL的基底节区脑出血患者46例,分为NGF+常规治疗组24例和常规治疗组22例。正常对照组20例,为同期健康体检者。用ELISA法动态测定血清中神经元特异性烯醇化酶(NSE)的水平以及随访6个月后进行ADL分级。结果常规治疗组患者血清NSE浓度在发病12h内至28d时均高于正常对照组(P<0.01)。NGF组患者血清NSE浓度在发病14d内高于正常对照组(P<0.01),21d以后与正常对照组比较无显著性差异(P>0.05)。NGF组患者血清NSE浓度在发病7d内与常规治疗组比较无显著性差异(P>0.05),但在14d以后与常规治疗组比较有显著性差异(P<0.01)。6个月后随访,两组ADL分级比较有显著性差异(P<0.05)。结论早期足量外源性NGF可以保护神经元,减少脑出血后的继发性损伤,促进神经功能恢复,改善患者日常生活能力。

Abstract:Objective To explore the effect of nerve growth factor(NGF) to the serum neuron-specific enolase(NSE) concentration in hypertensive intracerebral hemorrhage patients and clinical meaning.Methods 46 cases of intracerebral hemorrhage in which hematoma volumes were between 10ml to 30ml were selected and divided into NGF group(24 cases) and Conventional treatment group(22cases).The control group(20 cases) were healthy.Serum NSE was detected with ELISA method and the ADL grade were evaluated after 6 months.Results The concentrations of NSE in Conventional treatment group were obviously higher than that in control group from 12hour to 28day(P0.01).The concentrations of NSE in NGF group were higher than that in control group within 14days(P 0.01),but were no significant difference after 21day(P 0.05).The concentrations of NSE in NGF group were no significant difference than that in Conventional treatment group within 7days,but decreased rapidly and were obviously lower than that in Conventional treatment group after 14day(P 0.01).After 6 months follow-up,the ADL grade were significant differences between two groups.Conclusions Early enough exogenous NGF can protect neurons,reduce brain Secondary damage in the treatment of intracerebral hemorrhage,promote neural functional recovery,improve daily living ability of patients.
