卵巢卵泡膜细胞瘤16例超声误诊分析

目的分析卵巢卵泡膜细胞瘤的超声声像图特征及误诊原因,提高超声诊断水平。方法回顾性分析经临床手术及病理证实的卵巢卵泡膜细胞瘤16例的声像图及临床资料。结果 16例术前均行彩超检查,肿块的超声图像特征可分为两种:①盆腔低回声实性肿块12例,其中后方回声衰减明显者3例;②囊实性肿块4例。肿块内血流信号图像情况可分为三种:①内无血流信号4例;②内有少许血流信号10例;③内有较丰富血流信号2例。16例超声均误诊,提示为子宫浆膜下肌瘤4例,卵巢畸胎瘤1例,纤维瘤5例及未能明确诊断的卵巢肿物者6例。结论典型卵巢卵泡膜细胞瘤的超声图像表现有一定特征性,表现为包膜完整,内部呈均匀实性低回声,后方伴有或不伴有回声衰减,需密切结合临床病史进行诊断,以提高超声检查的阳性率。

Analysis of Ultrasonic Misdiagnosis of Sixteen the Comas

Objective To investigate sonographic features of ovary thecoma and analyse the reasons of misdiagnosis in order to improve diagnostic level. Methods Sixteen thecomas were retrospectively reviewed by the sonographic images and preoperation diagnoses. Results All sixteen patients with ovarian tumor were observed its colour Doppler ultrasonic features by preoperative ultrasonography.Colour Doppler ultrasonic features.of mass contain two： （1）12 were solid masses associated with lower echo pattern, 3were posterior echo attenuation,（2）4 were solid-and-cystic mixed tumors; Colorsigns of blood flow in mass contain three： Four mass weren＇t examined by color Doppler flow image,ten mass were showed a few colorsigns of blood flow,two mass were showed much colorsigns o f blood flOWo Sixteen thecomas all misdiagnosed,four subserous myoma,one ovarian teratom,Five ovarian fibroma and six were not diagnosed. Conclusions The solid ovarian thecomas with relativelytypical sonographic features,being regular in shape with lower echo pattern in solid mass and with posterior echo attenuation in it or not.So the combination of ultrasonographic appearance and clinical symptoms are helpfulin diagnosing thecoma may enhance the diagnostic accuraterate for thecoma ovarian.
