支架辅助栓塞大脑前动脉A1段微小动脉瘤

目的探讨支架辅助大脑前动脉A1段微小宽颈动脉瘤(动脉瘤最长径≤3mm,且动脉瘤颈/瘤体宽径比≥3/4)血管内治疗的可行性及效果。方法回顾分析Neuroform支架辅助栓塞10例12个大脑前动脉A1段微小宽颈动脉瘤的临床影像、血管内治疗和随访资料。结果 10例病人均在成功植入Neuroform支架后行动脉瘤栓塞,其中致密栓塞7例,3例90%栓塞,所有患者均随访6～24月无再出血,4例术后3～6月行CTA复查,6例于术后6～12月复查血管造影,未见再生长。结论大脑前动脉A1段宽颈微小动脉瘤的血管内治疗,技术上可行,操作相对安全,有效。

Endovascular treatment of tiny proximal (A1) anterior cerebral artery aneurysms with stent

Objective To investigate the feasibility and efficacy of endovascular treatment of tiny and wide-necked aneurysms（aneurysms ≤3 mm in diameter;and ≥3/4 in proportion of width of aneurysm neck and the maximal diameter）of the proximal （A1） anterior cerebral artery with stent.Methods Ten cases with 12 tiny proximal anterior cerebral artery aneurysms were embolized with Neuroform stent.The clinical,imaging,endovascular treatment and follow-up data were retrospectively analyzed.Results The aneurysms of 10 cases were embolized after Neuroform stents successfully implanted,and among aneurysms,complete occlusion is 7 cases 90% occlusion 3 cases.No re-bleeding symptom occurred during 6 to 24 months.Follow-up CT angiography in 4 patients after 3 to 6 months and digital subtraction angiography （DSA） in 6 patients after 6 to 12 months showed that no re-growth was found.Conclusion Endovascular stent is feasible,safe and effective in treatment of tiny and wide-necked aneurysms of the proximal （A1） anterior cerebral artery.
