全直肠系膜切除术保肛治疗低位直肠癌

目的 探讨低位直肠癌患者行全直肠系膜切除(Total mesorectal excision,TME)保肛术的临床疗效.方法 回顾性分析我科2006年1月至2011年1月42例行TME治疗低位结直肠吻合术的患者资料.结果 42例保肛手术成功,无1例出现局部复发及近期并发症,术后肛门括约肌功能正常,控便功能良好.2例2年后出现吻合口癌肿复发再次行手术治疗,1例3年后肝转移死亡.结论 TME保肛术治疗低位直肠癌安全有效,术后复发率低,并发症少,术后肛门功能良好,值得应用和推广.

Total mesorectal excision with anal sphincter preservation in the treatment of low rectal cancer

Objective To observe the clinnical efficacy in total mesorectal excision operation with anal sphincter preservation for low rectal cancer.Methods The clinical data of 42 patients with low rectal cancer undergoing total mesorectal excision operation with anal sphincter preservation were retroexpectly analyzed between January 2006 to January 2011.Results All sphinctersaving procedure were sucessful.Patients were followed with up for 6 to 60 months.There were no local tumor reocurence and later complications.The function of anal stenosis were good,but other two patients met with reoperation for the anastomotic tumor reocurence after two years,one died of hepatic metastasis after three years.Conclusion Total mesorectal excision operation with anal sphincter preservation is safe and effective in treating low rectal cancer.There are low rate of local tumor reocurence and complications.The function of anal stenosis were good.Total mesorectal excision operation should be used widely.
