大剂量（131）I治疗术后分化型甲状腺癌疗效及其影响因素分析

目的：回顾性分析大剂量131I治疗术后分化型甲状腺癌（DTC）患者疗效及其影响因素。方法：对47例DTC患者术后接受131I治疗的临床资料进行回顾性分析。对每次服131I后5～7 d131I全身显像进行对比观察作为判断疗效的依据,根据患者病理类型、转移部位、服131I次数分别观察其总体疗效。结果：大剂量131I治疗术后DTC患者总有效率80.9%,对于纵膈及颈部淋巴结转移的疗效明显优于肺、骨转移,无明显不良反应,安全可靠。结论：分化型甲状腺癌手术后加用大剂量131I治疗是一种安全有效的治疗方法,癌灶转移部位对131I治疗的疗效有一定的影响。

Influencing factors analysis on the clinical curative effect of differentiated thyroid carcinoma treated by using large dose of 131I

Objective: To perform retrospective analysis on the curative effect of large dose 131I therapy for treating post-operative differentiated thyroid carcinoma(DTC).Methods: Forty-seven post-operative DTC patients who had received 131I therapy were involved in this retrospective study.The efficacy was assessed by 131I whole body scan in five to seven days after the therapy every time.Besides,the overall curative effect was observed according to patients’ pathological type,location of metastasis and frequency of 131I therapy respectively.Results: The total effective rate of large dose 131I therapy was 80.9%.The curative effect for the patients with cervical and mediastinum lymph nodes metastasis was significantly better than that with lung and bone metastasis.Meanwhile,no obvious side-effects were observed during the study.Conclusion: Large dose of 131I therapy for DTC metastasis is a safe and effective.The location of tumor metastasis may influence the curative effect.
