转移性卵巢癌98例临床治疗及预后分析

目的：探讨转移性卵巢癌的临床治疗方法及影响预后的因素。方法：回顾性分析98例非生殖系统来源的转移性卵巢癌患者的临床资料；预后相关因素采用Cox比例风险回归分析。结果：98例患者治疗后中位生存期为8．7个月。0．5、1、5年生存率分别为57．1％、18．4％、1．0％；乳腺癌、结直肠癌、胃癌、黑色素瘤患者中位生存期分别为17．5、10．1、8．4、2．5个月。术前化疗及手术方式对预后影响不显著（P＞0．05），而原发癌是否根治性切除、术中残瘤灶大小、术后是否化疗明显影响其预后（P＜0．01，P＜0．01，P＜0．05）。结论：转移性卵巢癌预后较差，预后与原发癌期别及多脏器受累程度有关；但术中尽可能切除原发癌灶及腹腔转移癌灶使残瘤灶直径≤2cm，术后辅以化疗，仍可提高生存率。

Treatment and Prognosis of Metastatic Ovarian Cancer:Analysis of 98 Cases

Objective To study the treatment and prognostic factors of metastatic ovarian cancer.Methods The clinical data of 98 cases of metastic ovarian cancer from non reproductive systems was analyzed retrospectively.Results The median survival time was 8.7 months after treatment and the 0.5,1,5 year survival rates were 57.1%,18.4%,1.0% respectively,the median survival times of breat cancer,rectal and colon cancer,stomach cancer and melanoma were 17.5,10.1,8.4 and 2.5 months respectively.The regression analysis of single and multiple factors related to prognosis showed that the removal of primary tumor,residual tumor size after operation and chemotherapy after operation had significance( P <0.01?<0.01?<0.05).Conclusion The prognosis of metastatic ovarian cancer is poor,it relates to the stage of primary tumor,the number and extend of involved organs.The survival rate may be improved by resection of primary and metastatic tumors as much as possible,making the residual tumor size less than 2 cm,and chemotherapy after operation.
