腹腔镜胆囊切除术对胰高血糖素和C肽的影响

目的:了解腹腔镜胆囊切除术对胰高血糖素和C肽的影响。方法:20例择期在全麻下行胆囊切除术的病人,随机分为腹腔镜胆囊切除术组(LC组,n=10)和开腹胆囊切除术组(OC组,n=10)。分别于术前、气(开)腹后10min、术毕10min取血样,用放免法测定血浆胰高血糖素和C肽。结果:两组的胰高血糖素在气(开)腹后10min和术毕10min都高于术前,差异有统计学意义(P<0.05),其中OC组开腹后和LC组气腹后相比,升高明显(P<0.05)。LC组气腹后和术毕C肽水平均高于术前,术毕增高明显(P<0.05);和术前相比,OC组开腹后C肽降低(P<0.05),术毕升高。OC组开腹后和LC组气腹后相比差异有统计学意义。结论:LC对胰高血糖素和C肽的影响要明显弱于OC。

Influence of laparoscopic cholecystectomy on Glucagon and C-peptide

Objective To study the influence of laparoscopic cholecystectomy on Glucagon and C-peptide. Method 20 patients underwent elective cholecystectomy were randomly divided into laparoscopic cholecystectomy group(LC group, n=10) and open cholecystectomy group (OC group, n=10). Blood samples were obtained before operation, 10 min after pneumoperitoneum with carbon dioxide or laparotomy and 10 min after operation. The plasmic levels of C-peptide and Glucagon were measured with radio-immunoassay technique. Results Compared with preoperative values, significantly increased Glucagon were observed in both groups(P<0.05), but the value of Glucagon after laparotomy in OC group were much higher than that in LC group after pneumoperitoneun(P<0.05). The plasmic C-peptide levels after pneumoperitoneum and after operation were higher than those before operation in LC group and the level after operation significantly increased(P<0.05). Compared with preoperation, C-peptide level reduced after laparotomy(P<0.05) and increased after operation in OC group. The level of C-peptide in LC group after pneumoperitoneum has significant difference with that in OC group after laparotomy. Conclusion LC has a weaker impact on C-peptide and Glucagon compared with OC.
