腹腔镜与开放手术治疗腹股沟疝的对比研究

目的:对比观察腹腔镜腹股沟疝修补术与开放式无张力疝修补术治疗腹股沟疝的手术效果。方法:回顾分析2011年1月至2013年5月为65例腹股沟疝患者行腹腔镜疝修补术(腹腔镜组)的临床资料,其中40例行经腹腔腹膜前疝修补术,25例行完全腹膜外疝修补术;并与同期104例开放式无张力疝修补术(开放组)进行对比分析。结果:腹腔镜组术中失血量、疼痛评分及住院时间均优于开放组(P<0.05),两组并发症发生率差异无统计学意义(P>0.05)。两组患者均未见复发。结论:腹腔镜腹股沟疝修补术安全、可行,与开放手术相比,具有手术创伤小、术后康复快等优点;虽然手术费用高、操作技术难度大,但随着腹腔镜技术水平的提高及人们认识的改变,腹腔镜腹股沟疝修补术具有良好的发展前景。

A comparative study of open and laparoscopic inguinal hernia repairs

Objective：To compare the clinical outcomes between laparoscopic inguinal hernia repair （LIHH） and tension free open inguinal hernia repair. Methods： Clinical data of 65 cases of LIHR during the period from Jan. 2011 to May 2013 were retrospec- tively analyzed. Among which 40 cases were treated with transabdominal preperitoneal prosthesis, while the other 225 received totally ex- traperitoneal prosthesis. The data was contrasted with 104 cases of tension free open hernia repair during the corresponding period. Re- stilts：The blood loss, the score of postoperative pain and the average hospital stay in LIHR group were significantly less than those in open group （ P 〈 0.05 ）. There were no significant differences between the two groups in the postoperative complications （ P 〉 0.05 ）. No recurrence was observed in both groups. Conclusions： LIHR is safe and feasible with the advantages of more minimal operation trau- ma and faster postoperative recovery than open operation. Despite of the high operation cost and eomplex technology, LIHR has good de- velopment prospect along with the improvement of technical level and people＇ s knowledge.
