白细胞去除术联合化疗治疗高白细胞血症的疗效观察

目的 观察白细胞去除术联合化疗治疗高白细胞血症的临床效果.方法 选取本院自2010年1月至2015年4月收治的42例高白细胞血症患者作为研究对象,按就诊顺序编号,随机分为对照组与观察组各21例,对照组单用采用化疗干预,观察组则采用血细胞分离机作白细胞去除术后进行诱导缓解化疗,比较两组患者治疗效果.结果 观察组总有效率为71.43％,显著高于对照组的38.10％,两组比较差异有统计学意义(P＜0.05);治疗后,观察组血小板、血红蛋白水平无明显变化,但其白细胞计数明显降低,为(105.21±64.33)×109/L,明显低于对照组,两组比较差异有统计学意义(P＜0.05).结论 在高白细胞血症患者的临床治疗中,采用白细胞去除术联合化疗方案,可快速降低患者白细胞计数水平,整体疗效好,且不会增加患者出血风险.

Observation on curative effect of leukapheresis combined with chemotherapy in the treatment of hyperleukocytosis

Objective To observe the clinical effect of leukapheresis combined with chemotherapy in the treatment of hyperleukocytosis.Methods 42 cases of hyperleukocytosis admitted in our hospital from January 2010 to April 2015 were selected as study subjects,and randomly divided into control group and observation group with 21 cases in each group according to the order of treatment.Control group was only treated by chemotherapy,while observation group was treated by remission induction chemotherapy after leukapheresis with blood cell separator.The therapeutic effect was compared between two groups.Results The total effective rate in observation group was significantly higher than that in control group (71.43％ vs.38.10％;P＜0.05).After treatment,there was no significant changes in platelet and hemoglobin levels in observation group,but white blood cell count decreased significantly (105.21±64.33)× 109/L],which was significantly lower than that in control group (P＜0.05).Conclusion Leukapheresis combined with chemotherapy in the treatment of patients with hyperleukocytosis can quickly reduce the level of white blood cell count,with good overall effect,without the risk of bleeding increased.
