神经外科重症监护室院感危险因素分析及预防措施

目的 探讨神经外科重症监护室患者发生医院感染的相关危险因素,以指导临床工作,减少医院感染的发生.方法 回顾性分析400例神经外科重症患者临床资料,收集可能影响患者发生医院感染的危险因素,如年龄、住院天数、营养状况、手术情况、合并伤、是否气管切开、颅内压高低、床位设置方式以及合并多器官功能不全等详细资料,结合医院感染的发生情况进行统计学分析.结果 神经外科重症监护室患者医院感染与高龄、住院天数延长、营养不良、脑室引流、多次手术、合并其他脏器损伤、术前有合并症、气管切开、颅内压增高、大厅式集中管理以及合并多器官功能不全密切相关(P＜0.05).结论 高龄、住院天数延长、营养不良、术前有合并症、脑室引流、多次手术、合并其他脏器损伤、气管切开、颅内压增高、大厅式集中管理、合并多器官功能不全能是显著增加患者发生医院感染的独立危险因素.

Analysis of risk factors and preventive measures for nosocomial infection in NICU

Objective To investigate risk factors ofnosocomial infection in NICU patients,to guide the clinical work and reduce the occurrence ofnosocomial infection.Methods Retrospectively analyzed the clinical data of 400 critical patients in NICU.Collected risk factors which may influenced nosocomial infection,such as age,hospitalization days,nutritional status,operative situation,associated injury,tracheotomy,intracranial pressure,bed setting mode,multiple organ dysfunction syndrome (MODS).Carried out statistical analysis combined with the occurrence of nosocomial infection.Results Nosocomial infection in NICU patients was closely related to advanced age,long hospitalization days,malnutrition,ventricular drainage,multiple surgeries,combined with other organ injury,preoperative complications,tracheotomy,increased intracranial pressure,hall-style centralized management,MODS (P＜0.05).Conclusion Advanced age,long hospitalization days,malnutrition,preoperative complications,ventricular drainage,multiple surgeries,combined with other organ injury,tracheotomy,increased intracranial pressure,hall-style centralized management,MODS were independent risk factors of nosocomial infection in NICU patients.
