人工全膝关节置换与关节镜下滑膜切除治疗膝关节色素沉着绒毛结节性滑膜炎的膝关节功能和复发率对比

目的 对比研究人工全膝关节置换术(TKA)与关节镜下滑膜切除术(ASR)治疗膝关节色素沉着绒毛结节性滑膜炎(PVNS)的效果.方法 将88例PVNS患者随机分为A组与B组,各44例.A组行TKA治疗,B组行ASR治疗,对比两组术后膝关节功能及复发率.结果 A组术后ROM、Lysholm及IKDC评分均显著高于B组(P＜0.05);A组术后无手术并发症,与B组的4.55％比较,差异无统计学意义(P＞0.05);A组术后无复发病例,显著低于B组的11.36％(P＜0.05).结论 TKA治疗PVNS的近期疗效优于ASR,术后膝关节功能恢复更好,并发症更少且复发率较低.

Total knee arthroplasty versus arthroscopic synovium resection for pigmented vilionodular synovitis

Objective To explore the effect of total knee arthroplasty (TKA) versus arthroscopic synovium resection (ASR) for pigmented villonodular synovitis (PVNS).Methods 88 patients with PVNS were randomly divided into group A and group B,44 cases for each group.Group A were treated by TKA and group B ASR.After the operation,the knee function and recurrence rate were compared between these two groups.Results After the operation,the scores of ROM,Lysholm,and IKDC were significantly higher in group A than in group B (P ＜0.05).The incidence of surgical complications was 0.00％ in group A and 4.55％ in group B,with no statistical difference (P＞0.05).The recurrence rate was 0.00％ in group and was 11.36％ in group B,with a statistical difference (P＜0.05).Conclusions TKA in the treatment of PVNS has better short-term effect,knee function recovery,fewer complications,and lower recurrence rate than ASR.
