摘    要:目的 探讨纤维蛋白原(FIB)、D-二聚体(D-D)和纤维蛋白原降解物(FDP)的检测在儿童过敏性紫癜继发肾损伤中的表达及临床意义.方法 选择我院100例过敏紫癜患儿作为研究对象,按照是否出现肾损伤将其分为40例紫癜性肾炎(HSPN组)、60例普通型过敏紫癜(HSP组),另外选择30例健康儿童作为健康对照组,对三组儿童血FIB、D-D和FDP进行检测并进行相关分析.结果 在FIB、D-D和FDP阳性率方面,HSP组、HSPN组和正常对照组的表达差异具有统计学意义(P<0.05);HSP组和HSPN组FIB、D-D、FDP浓度水平均明显高于正常对照组(P<0.01);HSPN组FIB、D-D、FDP浓度与HSP组比较,表达差异均具有统计学意义(P<0.01);HSPN组血FIB和D-D、FDP呈显著正相关(r=0.553 P=0.000;r=-0.358,P=0.003).结论 继发性纤溶可能参与儿童过敏性紫癜的肾损伤过程,检测FIB、D-D和FDP对临床合理应用抗凝治疗具有指导作用.

Significance of blood levels of FIB,D-D,and FDP in children with allergic purpura secondary to renal injury
Abstract:Objective To explore the expressions of fibrinogen (FIB),D-dimer (D-D),and fibrinogen degradation products (FDP) in children with allergic purpura secondary to renal injury.Methods 100 allergic purpura children were selected from our hospital as study subjects.According to whether having renal damage,they were divided into 40 cases of purpura nephritis children((HSPN Group) and 60 cases of common allergic purpura children (HSP Group).And 30 healthy children were selected as a control group.The levels of FIB,D-D,and FDP were detected and analyzed in all these 3 groups.Results There were statistical differences in the positive rates of FIB,D-D,and FDP between these 3 groups (P < 0.05).The concentrations of FIB,D-D,and FDP were significantly higher in the HSP group and the HSPN group than in the control group (P <0.01).There were statistical differences in the concentrations of FIB,D-D,and FDP between the HSPN group and the HSP group (P <0.01).FIB positively correlated with DD and FDP in the HSPN group(r=0.553,P=-0.000;r=-0.358,P=0.003).Conclusions Secondary fibrinolysis may be involved in the process of children's allergic purpura secondary to renal injury.Detecting FIB,D-D,and FDP play a guiding role in the clinical application of anticoagulant therapy.
