去氨加压素在体外循环心脏手术中的应用

目的观察去氨加压素(DDAVP)对心脏病患者心脏手术体外循环中凝血功能以及出血的影响。方法选取217例行各类心脏手术需行体外循环患者进行临床分组观察,观察组109例手术过程中使用DDAVP,对照组108例未使用DDAVP。观察两组患者用药前后凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)以及纤维蛋白原(FIB)水平的变化,并对术后24h内出血量进行统计。结果观察组在用药后1hAPTT明显较对照组缩短(P<0.01),PT、FIB水平在手术后1h差异无统计学意义(P>0.05),术后24h出血量观察组显著低于对照组(P<0.01)。结论 DDAVP能够有效改善体外循环患者的凝血功能,减少术后出血量。

Application of desmopressin during cardiopulmonary bypass in heart surgery

Objective To observe the effect of desmopressin （DDAVP） on coagulation and bleeding in patients with heart disease during cardiopulmonary bypass in heart surgery. Methods A total of 217 patients undergo-ing various types of routine cardiac surgery by cardiopulmonary bypass in our hospital were divided into two groups. The observation group （n=109） applied DDAVP during surgery, while the control group （n=108） did not use DDAVP. The prothrombin time （PT）, activated partial thromboplastin time （APTT）, changes in the level of fibrinogen （FIB） before and after treatment, blood loss in postoperative 24 h were observed and analyzed. Results In the observation group, the APTT 1 h after treatment was significantly shorter than that of the control group （P0.01）, but PT, FIB level showed no significant difference （P0.05）. The blood loss in postoperative 24 h of the observation group was signifi-cantly lower than that of the control group （P0.01）. Conclusion DDAVP can effectively improve the clotting function during cardiopulmonary bypass in heart surgery and reduce the amount of postoperative bleeding.
