| 摘 要： | 目的分析前列腺癌（PCa）与前列腺增生（BPH）的MRI和MRS表现，探讨两者对PCa和BPH的鉴别诊断。方法回顾性分析经病理和随访证实的31例PCa和41例BPH患者的MRI、MRS表现，其中26例行MRS检查。MRI观察前列腺的大小、形态、病变位置、信号特点及肿瘤侵犯范围；MRS观察枸橼酸盐（Cit）、胆碱复合物（Cho）和肌酸（Cre）的波峰和化学位移，并计算（Cho＋Cre）／Cit（CC／C）比值。结果①MRI表现：PCa主要位于周围带（29／31，94％），T2WI均表现为结节状或片状低信号；BPH主要位于中央腺体（39／41，95％），T2WI表现为弥漫性混杂信号（18例）、圆形或结节状的高信号（17例）和低信号（6例）。②MRS表现：BPH的Cit峰升高，CC/C比值（0．79±0．25）下降；PCa的Cit峰下降，Cho峰升高，CC／C比值（2．34土1．21）升高，与BPH比较有显著差异（t=4．327，P〈0．001）。结论MRI能很好地显示BPH和PCa的位置和信号特点，MRS能提供前列腺良、恶性组织的代谢信息，两者联合诊断有助于提高PCa和BPH的鉴别水平。
