大肠癌亲和酶标法检测雌孕激素受体及临床生物学意义

摘    要:目的探讨雌孕激素受体与结直肠癌病理、临床及生物学行为的关系。方法应用免疫组织化学的亲和酶标法检测53例结直肠癌的雌激素受体和孕激素受体。结果雌孕激素受体总阳性率为60%(32/53);女性和男性阳性率分别为75%(21/28)和44%(11/25),差异有显著意义(P<0.05)。管状乳头状腺癌受体阳性率为70%,高于印戒细胞和粘液腺癌38%(P<0.05);DukesB期受体阳性率为75%,高于D期33%(P<0.05);淋巴无转移者受体阳性率为85%,高于有转移者45%(P<0.05)。肿瘤体积<5cm3者受体阳性率为71%,高于≥5cm3者39%(P<0.05)。直肠癌受体阳性率为71%,高于结肠癌33%(P<0.05)。高分化癌受体阳性率为78%,高于低分化癌35%(P<0.05)。结论女性大肠癌发病与受体关系密切;受体作用失常参与大肠癌发病始于较早期阶段;直肠癌发病与受体密度增高关系更为密切。

Abstract:PURPOSE To investigate the relationship of estrogen and progesterone receptors to the pathology,clinical and biological behavior of colorectal carcinoma.METHODS The E2-HRP,Pg-HRP method was used to detect the presence of ER and PR.RESULTS ER positive rate was 60% (32/53),the female and male positive rates were respectively 75% (21/28 ) and 44% (11/25 ), the difference Was significant (P < 0. 05 ). In tubular papilloadenocarcinoma,the receptor positive rate was 70%,higher than the 38% of the signet ring and mucoid adenocarcinomas (P<0. 05). Staging: Dukes' B had a receptor positive rate of 75 %,higher than the 33% of the D cases (P<0. 05);cases without lymph node metastasis had positive rate of 85 %,higher than 45% of the lymph node metastasis negative cases;tumor size less than 5 cm3 was 85%,as compared to the 39 % of the cases with tumors>5 cm3 (P<0. 05). The receptor positive rate for rectal carcinoma was 78 %,higher than the 33% of colon cancer (P<0. 05). High differentiation was 78%,whereas the cases with low differentiation had only 35% (P<0. 05).CONCLUSIONS The occurrence of colon cancer was strongly related to receptors in the female;abnormalities in the function of receptors are factors in the development of colon cancer,especially in the early stages;rectal cancers are even more closely related to the higher concentration of receptors.
