椎管内畸胎瘤的MRI诊断

目的　探讨椎管内畸胎瘤的 MRI诊断价值。方法　本组共 6例 ,男 2例 ,女 4例 ,均经手术病理证实 ,术前全部行 MRI检查 ,两例行 CT检查。结果　畸胎瘤位于脊髓圆锥 3例 ,胸段 2例 ,颈段 1例 ;髓外硬膜下 4例 ,脊髓圆锥内 2例。 MRI示病变信号混杂 ,T1 WI、T2 WI均见脂肪组织的高信号 ,脂肪抑制序列高信号被抑制而呈低信号 ,4例病变内 T1 WI、T2 WI见低信号的钙化 ,CT检查 2例明确钙化灶的存在。 3例合并其它先天畸形。结论　椎管内畸胎瘤 MRI表现具有特征性 ,MRI检查有助于作出正确的定位和定性诊断。

MRI diagnosis of the teratoma in vertebral canal

Objective To investigate the MRI diagnostic value of the teratoma in vertebral canal. Methods The MRI manifestations of 6 cases (2 men,4 women) with teratoma in vertebral canal confirmed by operation and pathology were analysed retrospectively,among which 2 cases performed CT scan. Results Of 6 cases,3 were found in conus medullaris,2 in thoracal segment,1 in cervical segment;4 cases were subdural teratoma,2 in conus medullaris teratoma. Hererogeneous signals were revealed on MR imaging. Fat issue showed high signal on both T 1WI and T 2WI,low signal on SPIR;calcification in 4 cases showed low signal on both T 1WI and T 2WI,in which 2 cases were confirmed by CT scan;3 cases combined with other malformations. Conclusion MRI features of the teratoma in vertebral canal are specific,and MRI is a reliable method for the diagnosis of the teratoma accurately.
