卡前列素氨丁三醇生理盐水稀释液胎盘附着部位点状注射在前置胎盘剖宫产术中出血的应用

目的探讨卡前列素氨丁三醇生理盐水稀释液在胎盘附着部位点状注射预防及治疗前置胎盘剖宫产术中出血的临床意义。方法2010年1月至2012年12月在枣庄市妇幼保健院产科住院分娩的126例前置胎盘患者，随机分为三组：研究组：剖宫产术中应用卡前列素氨丁三醇注射液1mL＋生理盐水9mL稀释后，在胎盘附着部位出血点处点状注射，每次注射1mL；对照组1：剖宫产术中应用卡前列素氨丁三醇注射液1mL肌内注射；对照组2：剖宫产术中应用卡前列素氨丁三醇注射液1mL宫体注射，比较三组间手术时间、出血量、不良反应及子宫切除率等有无差异。结果研究组较对照组术中出血少、产后出血量少、不良反应发生率低，三组相比差异有统计学意义（P〈0．05），三组间手术时间、子宫切除率相比无统计学差异。结论卡前列素氨丁三醇生理盐水稀释液在胎盘附着部位局部点状注射系预防及治疗前置胎盘剖宫产术中出血的有效方法。

The Application of Diluent of Prostaglandin Tromethamine in Placenta Previa During Pperation

Objective To investigate the clinical significance of the diluent ofprostaglandin tromethamine in placenta previa during operation. Methods 126 samples delivered in our hospital from January in 2010 to December in 2012 were randomly divided into three groups. The clinical circumstances of both observation group and control group were analyzed. Results The amount of bleeding during operation,the amount of postpartum bleeding, adverse reaction rate of the observation group were lower than the control group with significant difference（P〈0.05）. No significant differences were noted among the three groups in operation time and the rate of hysterectomy. Conclusion The application of diluent of prostaglandin tromethamine in placenta previa during operation is an effective method to prevent or cure postpartum hemorrhage.
