急性脑梗塞和动脉粥样硬化患者的颈动脉B超对照研究

摘    要:目的探讨颈动脉粥样硬化(CAA)与急性脑梗塞(ACI)和动脉粥样硬化(AS)患者年龄、血压、血糖、血脂、尿酸、C反应蛋白(CRP)等的动脉粥样硬化危险因子之间的关系。方法采用B型血管彩色多谱勒检查的方法,研究86例ACI患者和88例非脑梗塞(NS)患者进行检查,并分析两组患者入院时各项临床观察指标与颈动脉B超结果之间相关关系。为了进一步分析颈动脉斑块的可能机制,所有病例根据颈动脉斑块的有无重新分组为斑块组和无斑块组,并比较重组后的各项观察指标的差异。结果年龄和CRP可能是CAA形成的最主要危险因子(P<0.001;P=0.04),而血压、血糖、血脂、尿酸水平与CAA的关系并不密切。作为CAA重要指标的内-中膜厚度(IMT)也是随着年龄的增长而增厚。结论CAA的发生机制可能有别于全身其他部位的AS。

Abstract:Objective To study the relation between carotid atherosclerosis(CAS) and common risk factors ofatherosclerosis including age, gender, blood pressure, uric acid, C-reactive protein (CRP),etc.Methods86 patients with acute cerebral infarction(ACI)and 88 age-matched cases of atherosclerosis (AS) werechecked by ultrasonography on carotid arteries, which ultrasonic findings were associated with the clinicalmanifestations. To further investigate the possible mechanism and risk factors for CAS, all cases wereregrouped into the two groups by the presence or absence of carotid plaque: plaque group and non-plaque group,and then the common risk factors of AS were contrasted again. Results Age and CRP might be the most importantrisk factors of CAS (P<0.001; P=0.04), however, gender, blood pressure and plasma levels of glucose, lipid,uric acid, blood pressure could not show significant association with CAS. As an important index for CAS,intima-media thickness of carotid artery was also found increased with age.Conclusion The mechanism of ASin carotid arteries may be much different than that in cardiac and cerebral arteries.
