两种胰岛素应用方法治疗初诊2型糖尿病的疗效观察

目的观察短期胰岛素泵持续皮下输注（CSⅡ）与预混人胰岛素30R常规2次皮下注射对初诊2型糖尿病患者的疗效差异。方法将110例2型糖尿病患者随机分为两纽，其中A组采用CSⅡ治疗，持续皮下输注诺和灵R，B组于早、晚餐前30min注射预混人胰岛素30R，比较治疗后两组血糖达到良好控制所需时间及治疗两周后初诊2型糖尿病的缓解率。结果CSⅡ组和预混人胰岛素30R常规2次皮下注射组均可有效降低血糖，CSⅡ组血糖达到良好控制时间明显快于预混人胰岛素30R常规2次皮下注射组，两组比较差异有统计学意义（P〈0．01）。两周治疗后，停用胰岛素一周，仅使用饮食控制及运动疗法，2型糖尿病缓解率CSⅡ组明显高于预混人胰岛素30R常规2次皮下注射组，两组比较差异有统计学意义（P〈0．05）。结论初诊2型糖尿病患者中短期应用CSⅡ降糖效果显著，安全性高，患者耐受性和依从性好。

Effect of two methods of insulin use on type 2 diabetes

Aim To comparatively observe the effect and safety of continuous subcutaneous insulin infusion（CSⅡ） and Novolin 30R on newly diagnosed patients with type 2 diabetes mellitus （DM）. Methods there 110 type 2 DM patients were randomly divided into groups A and B group,Novolin 30R was used in B group twice daily in morning and evening.CSII was used in A group. Drug dose was adjusted according to the level of blood glucose.The levels of fasting plasma glucose （FPG）,therapeutic time in the two group0s were compared. Results CSⅡ group and Novolin 30R group could decrease blood glucose efficiently,the target blood glucose time in group A was 3.0±0.9 days,and 6.9±3.2 days in Group B,showing significant differences （P〈0.01）.Two weks after treated with indulin the patients were treated with controlled diet lus exercise, the relieving rates in Group A was hgher than that of group B （P 〈0.05）. Conclusion Continuous subcutaneous insulin infusion is an effective and safe mehod for treatment of newly diagnosed type 2 DM patients.
