天鹅型记忆接骨器的研制及其对全髋关节置换术后股骨骨折的治疗

摘    要:目的:研制一种专用于人工髋关节置换术后股骨骨折治疗的天鹅型记忆接骨器(SMC)。方法:依股骨的解剖学及生物力学原理,设计了用于人工髋关节置换术后股骨骨折的SMC。临床应用3例,平均年龄为67岁,2例为假体远端处股骨骨折,1例为假体股骨粉碎骨折。术后不用外固定,作早期功能锻炼,结果:平均随访49个月,术后12-14周骨折愈合,SMC固定可靠,患扶拐下地行走2周后弃拐。假体稳定,无X线松动表现。Harris评分为92分,结论:SMC固定骨折后,能够达到抗剪、抗折、抗旋并不能持续记忆加压于骨断端,促进骨折愈合,有效地治疗髋关节置换术后的股骨骨折。

Abstract:Objective: To develop swan like memory compressive connector(SMC) for fixing femur fracture at the prosthesis after total hip replacement.Methods:A special SMC was used to treat femur fracture at the prosthesis after total hip replacement. Three cases, 1 male and 2 female, averagely 67 years old, were treated with SMC. Two cases of the fracture were at the tip of the prosthesis, 1 case at the part of the stem. Without the need of external fixation,the patients could exercise early. Results:The follow up was 49 months. After 12 to 14 weeks, the fractures were union and the synthesis were stable. There was no loosening of the prosthesis, the Harris grade was 92. Conclusion:The characteristic of its anti shearing, anti break, anti rotation and the stress field produced by the connector is effective in the treatment of the femur fracture at the stem.
