| 摘 要： | 目的：探讨腹腔镜胆囊大部切除术在老年急性坏疽性胆囊炎中应用的可行性。方法：回顾2007年1月～2012年5月间老年急性胆囊炎行腹腔镜胆囊切除术（LC）106例患者，所有患者术前均无肺部感染、6月内无急性心梗，无严重心肺等重要器官功能不全等手术禁忌证。其中13例老年急性坏疽性胆囊炎患者行腹腔镜胆囊大部分切除术，同期93例老年急性胆囊炎（包括部分急性坏疽性胆囊炎）患者行常规方法腹腔镜胆囊切除术，在手术疗效及并发症方面进行分析。结果：腹腔镜胆囊大部分切除术组在手术时间和手术并发症、中转开腹牢方面明屐要优于常规方法LC组（P〈0．01），在住院时间、住院费用方面并无明显不同（P〉0．05）。结论：腹腔镜胆囊大部切除术在老年急性坏痕件胆囊炎中的应用是安全可行的。

| Abstract: | Objective：To investigate the medical feasibility and clinical value of Laparoseopic subtotal eholecystectomy （ LSC ） in elderly pa tients with acute gangrenous cholecystitis. Methods： Clinical data of 13 elderly patients who underwent LSC and 93 elderly patients who underwent LC from JAN ,2007 to May ,2012 were analyzed. Results： LSC was obviously superior to the Laparoscopie eholeeysteetomy （ LC ） in the operation time, opera tion complications and the rate of transferring operation. On the other hand, it was not significantly different in the length of hospital stay and the hospitali zation expenses. Conclusion： LSC is feasible and safe for elderly patients with acute gangrenous cholecystitis. |
