多发性硬化患者生活质量状况及相关因素分析

目的研究多发性硬化(MS)患者的生活质量状况,探讨相关因素对患者的生活质量的影响,以减少患者伤残。方法对170例患者进行多发性硬化患者专用生活质量量表(MSQOL-54)、Hamilton抑郁量表(HAMD)和焦虑量表(HAMA)、简易智力状态检查量表(MMSE)、躯体残疾状态调查量表(EDSS)进行量表评分,分别对患者的抑郁、焦虑、认知功能及EDSS评分与MSQOL-54量表评分的相关性进行分析。结果患者抑郁发生率为37.21%,焦虑发生率为31.40%,抑郁合并焦虑发生率为15.12%。认知功能障碍发生率为32.14%。病程、情感及认知功能障碍对MS患者生活质量均有影响(P0.05);MS患者的神经功能缺失程度(EDSS)与生活质量呈负相关(r=-0.759 0,P=0.000 1);复发次数越多、病程越长的MS患者生活质量越差(P0.05);不同临床类型MS患者的生活质量之间亦差异有显著性(P0.01)。结论 MS患者生活质量受到多方面因素的影响。

Condition of quality of life and related factors of MS patients

Objective To study the quality of life in patients with multiple sclerosis（MS） and the related factors that affect the quality ol life of patients,to reduce patient disability. Methods 170 eases of MS were assessed by the scales of MSQOL-54, Hamilton HAMD and HAMA, MMSE, Kurtzke EDSS. Correlation analysis was made of depression, anxiety, cognitive impairment, EDSS scores and MSQOL-54 scores. Results The incidence of depression in patients was 37.21% ,anxiety 31.40% ,depression combined with anxiety 15.12% , cog- nitive impairment 32.14%. Course of the disease, emotional and cognitive impairment affected the quality of life of MS patients （ P 〈 0.05）. The EDSS scores had a significantly negative correlation with the scores of QOL（r = -0. 759 0,P =0.000 1 ） ;The more the relapse of the disease, the longer the duration of MS patients with worse quality of life （P 〈 O. 05 ） ;The qualities of life in patients with different clinical forms of MS were significantly different（ P 〈 0.01 ）. Conclusion There are various factors which affect the QOL of patients with MS.
