异体角膜缘干细胞移植联合结膜移植治疗严重眼表疾病

目的观察严重眼表疾病患者行异体角膜缘干细胞移植 结膜移植 结膜囊成形术的临床疗效。方法回顾性分析2003年1月到2007年12月在我院行异体角膜缘干细胞移植联合结膜移植及结膜囊成形术的36例患者临床资料,观察其视力变化、术后睑球粘连范围及干眼情况等。结果术后3个月异体植片全部成活,随访6~35个月,所有患者角膜血管翳均见萎缩变薄,结膜纤维化程度减轻。术前矫正视力最高为0.12,术后矫正视力最高达0.8。13例患者角膜病灶不同程度恢复透明,22例患者视力进步,19例患者睑球粘连缓解,24例患者自觉干眼症状好转。结论异体角膜缘干细胞移植联合结膜移植是治疗严重眼表疾病、改善眼表状况、提高视力、缓解干眼症状的一种有效方法。

Clinical analysis on treating severe ocular surface disease by limbal stem cell allograft and conjunctiva transplantation

Objective To observe the curative effect of limbal stem cell allograft and conjunctiva transplantation associated with fornix reconstruction to treat severe ocular surface disease in 36 cases.Methods Retrospective review was taken at 36 patients who underwent limbal stem cell allograft and conjunctiva transplantation associated with fornix reconstruction from January,2003 to December,2007.The changes of visual acuity,conglutination extent from eyelid to eye ball,dry eye conditions were obsorved after treatment.Results All the allografts survived at the three months postoperatively.Corneal pannus were atrophy and conjunctiva fibrosis decreased following up from 6 to 35 months.The best pre-operative corrected visual acuity was 0.12 and the best post-operative corrected visual acuity was 0.8.Cornea of 13 cases became partly transparent,visual acuity of 22 cases improved,symblepharon of 19 cases decreased,and dry eye symptom of 24 cases relieved.Conclusion Limbal stem cell allograft and conjunctiva transplantation associated with fornix reconstruction is an effective treatment for patient with severs ocular surface disease.It can improve the ocular surface condition,progress the vision acuity,and relieve dry eye symptom.
