基层医院开展腹腔镜下腹会阴联合切除术的效果研究

目的比较腹腔镜与开腹直肠癌腹会阴联合切除(miles)术在直肠癌治疗中的应用效果。方法比较同一手术组所行的腹腔镜及开腹直肠癌miles术相关指标,包括手术相关指标、肿瘤根治性指标及医疗费用。结果共有30例行miles术:14例行腹腔镜手术,16例行开腹手术。腹腔镜组在手术相关指标、根治性指标方面与开腹组相比无显著性差异(P>0.05)。腹腔镜组手术费用高于开腹组(P<0.01),但2组在总体费用方面却无显著性差异(P>0.05)。结论腹腔镜下miles术在根治的同时,无明显增加患者经济负担,可能更易在基层医院推广。

Titer study of laparoscopic miles operation in matrical hospital

Objective It is to compare the effectiveness of laparoscopic and open miles operation on rectal cancer.Methods The related indexes of laparoscopic and open miles operation on rectal cancer in the same operation group were compared.The indexes included operation related index,tumor radical index and medical cost.Results 30 cases were treated with miles operation,in which there were 14 cases of laparoscopic operation and 16 cases of open operation.There was no significant difference on operation related index and radical index between laparoscope group and open group(P>0.05).The operation cost in laparoscope group was higher than that in open group(P<0.05),but there was no significant difference on total cost between the two groups(P>0.05).Conclusion At the same time radical cure,laparoscopic miles operation does not obviously increase economical burden of patients.Laparoscopic miles operation may be easier for the generalization in matrical hospital.
