CT血管造影对颅内肿瘤的鉴别诊断价值--附8例报道

摘    要:对 8例疑动脉瘤可能或合并动脉瘤患者应用ElscintCT Twin扫描机行CTA检查。CTA条件为Pitch 1 0 ,层厚 1 3mm ,层间距 0 8mm ,非离子型碘剂 80~ 10 0mL ,注速为 3 0mL/s ,延迟扫描时间为 15~ 2 2s。采用MaxIP、MPR和SSD进行编辑后处理。 8例经CTA检查均排除了动脉瘤 ,其中 4例垂体瘤 ,3例脑膜瘤 ,1例颅咽管瘤 ,CTA诊断后均经手术病理证实。初步研究的结果提示 ,如疑颅内病变为动脉瘤或肿瘤同时合并有动脉瘤 ,可用CTA进行鉴别。除了能排除动脉瘤外 ,利用CTA原始图象和三维编辑图象还能多方位显示肿瘤确切位置、起源和与相邻结构关系 ,术前多能明确诊断。

Abstract:Eight patients with intracranial tumor were suspected aneurysms or tumor combining with aneurysms. All the patients (n=8) were examined by CTA. The CTA scans were obtained by Elscent CT-TWIN,(pitch 1.0, collimation 1.3 mm, increment 0.8 mm, scan delay 15-22 sec) and noniomic idolinated contrast material (80-100 mL) was injected intravenously at a rate of 3.0 mL/sec, and then the source images were processed with MaxIP, MPR and SSD reconstruction. It was demonstrated that none of cases with CTA were aneurysms. All the patients included patients with pituitary adenoma (n=4), meningioma (n=3) and craniopharyngioma (n=1). All of the CTA diagnosis were proved by surgical and pathologic findings. The preliminary study suggests that CTA enables us to recognize intracranial aneurysm or intracranial tumor combining with aneurysm. In addition, the original and 3D reconstruction images of CTA can provide the accurate position, origin of the tumor and the relation with contiguous tissue and make dependable diagnosis before surgical operation.
