2型糖尿病合并高尿酸与颈总动脉内膜厚度相关性研究

目的：探讨2型糖尿病高尿酸血症与颈总动脉内膜厚度的关系。方法：将2008年10月至2009年11月青岛内分泌糖尿病医院收治的267例2型糖尿病（T2DM）患者按血尿酸水平分为高尿酸组及正常尿酸组，比较两组一般临床资料及颈总动脉内膜厚度，并进行临床分析。结果：与2型糖尿病正常尿酸血症组相比，糖尿病高尿酸血症组的体重指数（BMI）、腰臀比（WHR）、舒张压（DBP）、三酰甘油（TG）、空腹胰岛素（FINS）、胰岛素抵抗指数（HOMA—IR）、肌酐（cr）水平明显升高，而高密度脂蛋白胆固醇（HDL—C）水平降低。Logistic回归分析显示，DBP、Cr、TG、HDL—C、FINS、HOMA—IR是2型糖尿病高尿酸血症的危险因素。结论：2型糖尿病患者中，随着尿酸水平的增加，IMT亦增加，高尿酸血症加重了2型糖尿病的代谢紊乱，尤其是对大血管并发症起促进和加重作用。

Relationship between Type 2 Diabetes Mellitus Combined with Hyperuricuria and Common Carotid Artery Intima-Media Thickness

OBJECTIVE： To investigate the association between hyperuricemia combined with type 2 diabetes mellitus and common carotid artery intima - media thickness. METHODS ： 267 patients with type 2 diabetes mellitus enrolled in Qingdao endocrinology and diabetes hospital during the period from October 2008 to November 2009 were divided into two groups including hyperuricemia group and normal uric acid levels group according to their serum uric acid levels. Serum uric acid levels and common carotid artery intima - media thickness in two groups were compared and analysed. RESULTS ： Compared with uricemia group, hyperurieemia group showed higher levels of body mass index （BMI） , WHR, DBP, triglyeeride （TG） , FINS, HOMA - IR and Cr and lower level of high density lipoprotein - cholesterol （HDL - C）. Losgistie multivariate analysis showed that DBP, Cr, TG, HDL - C, FINS and HOMA - IR were the risk factors for hyperuricemia in elderly patients with type 2 diabetes mellitus. CONCLUSIONS ： Among the patients with type 2 diabetes mellitus the levels of uric acid and common carotid artery intima - media thickness are increased. Hyperuricemia increases the severity of metabolic disorders in patients with type 2 diabetes and especially aggravates macrovaseular complications of type 2 diabetes.
