某部新兵病毒性肝炎爆发流行与临床初步研究

摘    要:目的:查明新兵中以单项血清转氨酶增高为主要特征的肝损害患者的病因、传播途径和临床特点。方法:对207名2001年入伍新兵进行流行病学调查,血清和粪便进行肝炎病毒标志物检测,肝组织活检以及临床特点分析。结果:根据流行病学调查和临床检测结果,这些病人可排除各种中毒因素,已知肝炎病毒的血清标志物均阴性,均无症状体征,肝组织活检肝细胞普遍肿胀变性,点状坏死,汇管区炎症细胞浸润,1个月内转氨酶升高的新兵占新兵总人数的36.23%,从病人血清和粪便中可以检出TTV DNA。结论:本次肝炎流行可能是由TTV引起的,属非甲~庚型病毒性肝炎,传播方式可能是TTV污染碗筷引起的消化道传播。

Abstract:Objective: To investigate the cause morbi, route of transmission and clinical features in patients with injury of liver who were mainly charaterized by elevation of serum single transaminase in recruits. Methods: Epidemiology was investigated, viral heaptitis markers of sera and feces were detected in 207 recruits enlisted in 2001. Clinical features of patients were analysed, and liver biopsy was performed in part of them. The results: Based on epidemiological investigation and clinical detection results, toxic factors were excluded in these patients. Well-known sera viral hepatitis markers were negative. No symptome and signs were found. Results:of liver biopsy showed general swelling and degeneration of liver cells,spotty necrosis,and inflammatory cell infiltration in portal area. Recruits with elevated transaminase within a month accouted for 36. 23% of the total recruits. Conclusion: NonA-G type hepatitis epidemiogenesis in this time may be caused by TTV. The mode of transmission may be digestive tract transmission caused by TTV infecting bowls and chopsticks.
